I am submitting, signing and delivering the foregoing documents as the parent/guardian on behalf of my child and in my individual capacity. College Basketball Prospects of America, LLC (“Prospects”) and I have agreed to conduct the submission of such documents, and my signing and delivery of such documents by electronic means. I acknowledge and agree that the subject documents, and all other pertinent information, have been provided, sent and delivered to me in an electronic record capable of retention by me at the time of receipt, and that neither Prospects nor its information processing system has inhibited my ability to print or store such electronic record.
